Senior Band, Senior Jazz Band & Chamber Choir Trip Packages
Dear Senior Band, Senior Jazz Band & Chamber Choir students & families,
Please see the updates regarding the Edmonton Trip that students received in class; one day December Field Trip package AND the Edmonton Multi-day Field Trip Package. Forms for both trips must be submitted before students can go on any of the trips.
IMMEDIATE ACTION NEEDED FOR EDMONTON TRIP: As outlined in the letter, we need to collect traveler’s information to secure out booking for flights. Students who wish to take part on the Edmonton trip must complete the following form: https://forms.office.
We need to collect information by Wednesday, Nov 30. Please review the informed consent package for travel details over the weekend. I have attached the main Informed Consent Package in this email as well for your information (Please submit the forms distributed in class and refrain from printing the attached package. We color coded the forms to help us with organization!)
Prior to submitting the Teams Form, please ensure that the information submitted matches student passport information as this will be used to issue the flight ticket. Additional charges will incur for any changes made after the initial submission.
